If you are interested in becoming a 212 Exchange Street service provider, submit your company information to see if you qualify. If selected to become a 212 Service Provider, you will be required to fill out/submit the following information:
An independent contractor agreement
Proof of the following minimum insurance coverages:
Automobile insurance of at least $1mm
Umbrella/excess liability insurance of at least $1 million per occurrence
Workers Compensation insurance of at least $1mm per occurrence
General Liability Insurance of at least $1 million per occurrence and $2mm in the aggregate, with the following minimum sub-limits:
$100,000 for damage to rented premises
$5,000 medical expenses per person
$1 million personal and adv injury
$2 million products – comp/og agg
All of this is fairly standard and simple for your insurance agent or broker to handle. 212 would be happy to communicate directly with your insurance agent or broker to arrange if needed.

A business questionnaire (provided by 212)
Your agreement to comply with 212’s Code of Conduct (provided by 212)
